Mr. Cordell Nakia Forman was born on November 20, 1986, in Gadsden, Alabama, to Ms. Ivory Crayton and the late James Forman.
He confessed his love for Christ at an early age and attended The Tabernacle Church in Gadsden, Alabama.
He was proudly educated at Carver High School. He then later attended Lawson State Community College to further his education.
He departed this life on December 2, 2018, in Gadsden, Alabama.
He leaves to cherish his precious memories, his daughter: Nakia Forman; his son: Ezra Williams; his sister: Marjorie Reed; sisters: Kristie Blount, Delta Thomas and Sebrenna Graves; brother: Patrick Blount; aunt: Pamela Butler; uncles: Nick Clifton, Stanley Bradford; John Hammock and Eric Hammock; grandmother: Barbara Clifton; special cousins: Dominique and Charda Butler, Tylie K. and Deonte' Harvey and Caleb Tarver; a special niece: Sparkel Brown; a special friend: Tiffany Matthews; and a host of relatives and friends.
